The IHS must be paid even if you have your own private medical insurance and do not intend to use the NHS.     The UK tourist visa allows you to stay up to 180 days in the UK. But if you stay that long continuously in the UK on a tourist visa, you will have serious trouble getting another UK visa in the future or entering the UK next time.
